Boistfort Valley Farm CSA: Sign up!

Please fill out this form, print it & mail it with your check to
apply for membership in our CSA program.

* Cost of Family Share is $600, or $575 if you sign up before May 1st!
* Cost of Small Share (feeds 2) is $480, or $460 if you pay by May 1st.
* Produce is packed in boxes and delivered to a central location in your
  neighborhood. Please return boxes in usable condition from the
  previous week when picking up your next delivery.
* Payment schedule is based on 20 weeks. After receipt of your deposit,
  payments are due the first of each month beginning beginning July 1st.
  If payment is not received by the 15th, deliveries will be suspended.
* No refunds will be granted after July 31. Refunds may be subject to
  cancellation fees.
* Late membership will be prorated.
