Please fill out this form, print it & mail it with your check to
apply for membership in our Holiday CSA program.

* Cost of CSA is $65 singly, or $130 for two Monthly deliveries.
* Produce is packed in boxes and delivered to a central location in your
  neighborhood. Please return boxes in usable condition from the
  previous month when picking up your next delivery.
* Please note that all South end deliveries (Tacoma and points south)
  will take place on Wednesdays. Seattle area deliveries will take place
  on Thursdays. Boxes will be delivered on Wednesday, November 17th and
  Thursday, November 18th and Wednesday, December 15th and
  Thursday, December 16th.
